Student Wellness
The CHIP Center team connects students to local health and wellness organizations in the Buffalo, NY community. For acute medical needs, students can visit the CHIP Center's Wellness Kiosk in Yurtchuk 116 for access to free wellness and over-the-counter (OTC) products.
For more emergent medical needs, students can visit several urgent care centers within 2 miles of Amherst's campus. Additional resources are listed below by category. To discuss options on a 1:1 basis, students are encouraged to visit or contact the CHIP Center Yurtchuk Student Center (1st floor), email chip@daemen.edu, or call +1 (716) 839-7380 (M-F, 8:30 am - 4:30 pm) to make an appointment for resource connection.
The Wellness Kiosk is a free, self-service resource to all students, available during office hours, M-F, 8:30am - 4:30pm. Out-of-Pocket Costs & Insurance
To be seen, payment is required at the time of service and could include but is not limited to a copay (usually $20 – $75*). Major insurance plans are accepted** at urgent care centers to assist with medical costs however, patients may receive a bill after their visit for services provided.
If your insurance is NOT accepted, you may be required to pay out-of-pocket costs to be seen that can range from $100 - 250*. Some NYC-based plans (MetroPlus, HealthFirst, etc.) and out-of-state Medicaid plans may be denied. Check the back of your insurance card for details on submitting a claim after your visit.
Transportation
Students can call 716-839-7233 [SAFE] or stop by Campus Safety (Yurtchuk Lobby) for assistance with transportation to urgent care.
Student Support
If a student has medical documentation noting they will be physically out of classes, they are encouraged to submit a care report at daemen.edu/concern. For additional information, please review the section below: Medical Documentation & Academic Support.

Students that need to be out of classes for 3 or more days for medical or mental health reasons should contact the CHIP Center team at +1 (716) 839-7380 or email chip@daemen.edu, and/or submit a CARE report at daemen.edu/concern to put formal support in place.
The CHIP Center team will need a copy of documentation from the provider(s) with the length of time the student needs to be out of classes*, a list of restrictions (if applicable, including but not limited to things that will impact their physical, mental or academic engagement) and any other information that would be helpful to support the student.
- A copy of medical documentation can be submitted online to daemen.edu/healthupload or faxed to 716-839-8230. Students are encouraged to keep an original copy for their records.
- A copy of medical documentation can also be submitted through the CARE report system if the student decides to use that resource for reporting.
Academic Support & Engagement
Once appropriate documentation is received, a prolonged absence notice (formal letter) will be sent from the Registrar's office to the student and their registered academic instructors. The letter will note that the student is out of classes for a medical reason through a specified date (if known) and the student's ability to participate (virtually, not at all, etc.).
In addition to the prolonged absence notice, students are responsible for contacting their instructors, engaging in their classes, and maintaining communication with the instructors and the CHIP Center should their status change.
Students with an acute illness or injury may have the right to additional academic support and accommodations. We strongly encourage students who may be in need of accommodations to connect with Accessibility services ASAP.
*If a student needs to be out of classes for a length of time that exceeds 2 weeks, the student should contact the Student Success Center to discuss options for academic support beyond a prolonged absence notice.
Tips on Finding a Therapist
- Start thinking about your goals for therapy! Determine what you want your life to look like and what barriers are in the way. Also consider if there are any traits that you are looking for in a therapist. You are allowed to have a preference on gender, culture, religion, etc.
- Find out who accepts your insurance! Once you have an idea of what you want to see a therapist for, check who accepts your insurance. A few ways to do this are utilizing psychologytoday.com and filtering results based off of location and insurance plan, or using your insurance companies website to find all in network providers.
- Start emailing or calling! We advise reaching out to 3-5 therapists that you think could be a good fit to see if they are accepting new patients. Have your insurance card ready as they may request information on the card. Give them a few days to respond, if you don’t hear from them within a week, follow up or reach out to a few more.
Other Considerations
- Before you attend your first appointment, you can ask what the cost per appointment will be.
- Ask if they offer a sliding scale, lower income may equate to less expensive appointments.
- Don’t be discouraged if your first therapist isn’t the right fit for you. Try to give it at least three to five sessions before deciding to find a new provider.

Tips on Finding a Primary Care Provider
- Find out who accepts your insurance! Call the number on the back of your insurance card, or utilize their website to find a list of in-network providers. If calling, once you reach a representative on the phone, here are some examples on things to say:
- “ I am looking to find a primary care provider in my area. Can you provide a list of in-network providers?”
- “Will I have a copayment, and if so, how much will it be?” A copayment is what you payout of your own pocket for every medical visit.
- Decide what you want in a PCP! You know yourself best. It is important to make sure that you receive the care that you are looking for.
- Do you want a PCP who speaks a certain language?
- Do you have a car? Does the office need to be in close proximity to where you live?
- Is the age, sex, race, or religion of the PCP important to you?
- Call the PCP Office for more information. Here are some things to ask or say when you call:
- Are you taking new patients? Do you take my insurance?
- How soon can I make an appointment?
- When booking an appointment, make sure to give all of the necessary information. (name,date of birth, insurance information, reason for your visit)
- Confirm the date and time of the appointment and write it down. Don’t forget to also write down the name of the PCP, their phone number, and their address.
- Prepare for your visit.
- Make a list of questions you want to ask the PCP.
- It is your legal right to have an interpreter (a person who translates) if you need one. Ask for an interpreter if you do not speak English well and your doctor does not speak your language.
- After the visit.
- Make sure that you understand everything the PCP said and that all of your concerns were addressed.
- Schedule any necessary follow up appointments, as discussed with your PCP.
- Ask about their office’s use of an online portal for future communication and appointment making.
